TERMS OF ENGAGEMENT & INFORMED CONSENT FORM
Good nutrition supports the body's natural defences and resistance. However, no claim can be made about the efficacy of any naturopathic nutritional advice.
1. Scope of Nutritional Therapy
Tailored Support: Nutritional advice will be tailored to support medically diagnosed conditions and/or health concerns agreed and identified by both parties.
No Medical Diagnosis: Nutritional therapists are not permitted to diagnose or claim to treat medical conditions. Nutritional therapy is not a substitute for professional medical advice, diagnosis, or treatment.
Medical Oversight: You are responsible for contacting your GP or specialist about any health concerns you may have. Please advise your GP of the naturopathic nutrition protocol you will be following, as well as any other complementary medicine practitioners you are consulting.
2. Client Responsibility & Full Disclosure
Honest Disclosure: It is important that you tell your practitioner about any medical diagnosis you have received, any prescription medication, herbal medicine, food supplements, or over-the-counter medication you are taking, as it may affect or conflict with the naturopathic nutritional program. 
Protocol Timeframes: Your naturopathic nutritional program and supplement plan will have a specific timeframe. You must not continue with recommendations outside of this timeframe unless explicitly agreed by your nutritional therapist.
Immediate Cessation & Adverse Reactions: The Client acknowledges that due to biochemical individuality, the body may occasionally experience unexpected sensitivities to new dietary variations or nutritional supplements. In the event that the Client experiences any adverse reactions, discomfort, or unexpected physical symptoms, the Client must immediately cease taking all recommended supplements or engaging in the specified wellness activities, and seek immediate guidance from their primary healthcare provider (GP) or emergency medical services. The Client must report any concerns or questions about their program to their nutritional therapist via email. The Client agrees to notify Nutri by Juste of any protocol cessation due to adverse reactions as soon as reasonably practical.
3. Working Hours & Communication Boundaries
Operating Hours: The practitioner reviews and replies to messages during standard business hours (Monday to Friday, 9:00 AM – 5:00 PM). Messages received outside of these hours, over weekends, or during UK bank holidays will be processed when the service re-opens.
Practitioner Holidays & Annual Leave: The Client acknowledges that the practitioner is entitled to scheduled annual leave and occasional service closures. The operational exemptions detailed in this section apply in full during holiday closures, and all correspondence will be processed in chronological order upon the practitioner's return.
No Emergency Service: The practitioner does not guarantee immediate response times and does not provide an out-of-hours emergency or medical crisis service. If you require immediate health clarification, you must use your email to state your query clearly, allowing for a standard administrative review window. In the event of an urgent medical crisis during a holiday or standard closure period, the Client must contact emergency services or their GP immediately.

4. Functional, Laboratory & Nutrigenomic Testing

All-Inclusive Scope of Testing: The Practitioner may recommend a wide range of private laboratory investigations. This includes, but is not limited to, any and all functional biomarker testing, nutrigenomic profiles, biochemical assessments, or tissue and cellular panels utilizing any biological sample. All such investigations are utilized strictly to evaluate nutritional requirements and/or metabolic individuality. They are wellness and preventative tools and are not utilized to diagnose medical conditions or active diseases. The information provided in reports should not be used for diagnostic or treatment purposes and is not a substitute for personal medical advice. Use the information provided solely at your own risk.

Medical Interpretation: If any test result indicates a marker that falls significantly outside of normal parameters or suggests a potential underlying medical condition, the Practitioner will explicitly direct the Client to present the raw data to their GP or a medical specialist for formal clinical evaluation.
Test Fees & No-Refund Policy: All testing kits are external to the consultation fees. Unless otherwise agreed, testing fees are paid by the Client directly to the relevant laboratory provider. Where the Practitioner explicitly agrees to order a test kit on the Client’s behalf, the Client must pay the Practitioner in full before the order is placed. Once an order is submitted to the laboratory, testing fees are 100% non-refundable and non-exchangeable under any circumstances, as third-party laboratories do not issue refunds or cancellations to the practitioner.
Laboratory Liability: The Practitioner assumes zero liability for laboratory delays, processing errors, damaged test kits, or shipping faults caused by third-party testing facilities.


5. Commercial Terms, Fees & Cancellations
Cancellation Policy: The practitioner operates a strict 24-hour cancellation policy. Appointments rescheduled or cancelled with less than 24 hours' notice, or missed entirely ("no-shows"), will be charged at the full standard consultation rate or forfeited from your pre-paid package.
Package Expiration: To maintain clinical momentum and ensure safety, all multi-session packages must be utilized within 6 months of the date of purchase. Any unused sessions beyond this period will automatically expire and are non-refundable.
No Guaranteed Outcomes: The Client acknowledges that individual health outcomes depend on numerous variable factors, including genetic individuality, pre-existing conditions, and strict adherence to the agreed dietary and lifestyle protocols. While the Practitioner will apply full professional expertise, the Practitioner does not guarantee specific health outcomes or biological results. Fees are paid strictly for the Practitioner’s clinical time, research, and expert guidance; refunds are not issued based on a client's subjective results.
6. Termination of Services
The Practitioner reserves the right to terminate the professional relationship and cancel any remaining sessions within a package immediately if the Client is consistently non-compliant with safety protocols, fails to provide an honest medical history, repeatedly breaches communication boundaries, or displays abusive behaviour. In the event of termination by the Practitioner, a pro-rata refund will be issued strictly for any remaining, unused consultations.
7. Data Protection & Record Retention (UK GDPR)
The Practitioner acts as an independent Data Controller registered with the Information Commissioner's Office (ICO). Personal and sensitive health data is processed strictly in compliance with the UK GDPR and the Data Protection Act 2018.
The Client acknowledges that due to professional indemnity insurance mandates and clinical liability regulations, the Practitioner is legally required to retain clinical case notes and health records for a minimum statutory period (typically 7 years from the date of the last consultation), and this regulatory requirement supersedes a client's GDPR 'right to erasure' request during this timeframe.
8. Electronic Acceptance
The parties agree that digital acceptance of these terms—including ticking a consent box upon booking via an online scheduling portal, or signing electronically via digital signature software—shall carry the full legal weight and validity of a handwritten signature.
